Please print out all of the following - sign and mail -
Signed forms MUST be received a week in advance before any therapeutic lessons will be scheduled !
For Standard Riding Lessons, you will need the forms signed - release form page 1 and 2, medical authorization, and the rules and reminder page. The files here are available as PDF or word format.
If you wish to provide your own helmet - please print and sign Helmet Release form - and please be aware that helmets need to be replaced every other year !
THANK YOU for downloading forms - saves us printing and postage costs - However if you have troubles - we are happy to mail you a packet ~
When your forms are printed out and signed - mail them to the address below.
Dream On Curls is a facility located on the property of a private residence, forms cannot be dropped off.
